Seeking Instructors at World Wide Marine Training for STCW BASIC TRAINING (Oriental, NC location) and OUPV and Upgrade to Master 100-courses for the Midwest locations of Grafton, Il,  St. Louis, MO and Lake of the Ozarks , MO. Instructors teaching the OUPV and Upgrade classes must hold Near Coastal credentials matching these courses.

FCC now requires applicants to register an FCC profile and FRN before taking the MROP Element 1 exam

Remember this has to be done BEFORE registering for MROP exam!

MROP: Required on vessels over 300 gross tons and vessels that carry more than 6 paying passengers

MCA – MARINER CREDENTIAL ASSISTANCE – $125.00

Let World Wide Marine Training assist you with your application package.  We will review your documents and submit them electronically through the National Maritime portals. 

Below you will find all the current Coast Guard forms you will need to apply for your license. 

If you Do Not have any convictions since your last issuance, disregard the Conviction Discloser Form. 
A complete list for your application is as follows:

Application Package Requirements:

  1. Application CG-719B  Sign page 4 as applicant. Sign page 5 if we will be submitting your application. Your signature being notarized no longer applies.
  2. Conviction Disclosure Statement if you answered “YES” to anything on page 2 of the Application – Click Here
  3. Pay.gov receipt for type of credential you are applying for – Click Here
      1. License – Original and Raise in Grade (RIG)
      2. MMD – Original and Raise in Grade (RIG)
      3. Renewal & Modification of Scope 
  4. Physical CG-719K – to be filled out by your doctor.
  5. Entry Level Physical (For Original Ordinary Seaman, Wiper, FH) – CG-719KE
  6. Drug Test CG-719P
  7. TWIC (Transportation Workers Identification Card) – (Front & Back)
    To Enroll call #855-347-8371
  8. Copy of your current credential (Renewal or Raise in Grade or Modification)
  9. Copy of Birth Certificate OR Passport
  10. Sea Time documented on Small Sea Service Vessel Form CG-719S – Or a Detailed letter from a Marine Employer – One form for each vessel used. If the vessel is yours, a copy of its registration as well. At least 360 days, 90 of which are to be within the last 3 years.
  11. Copy of your Course Certificates of Completion
  12. Current First Aid/CPR Card (Original License Only) – First Aid course has to be taken within 12 months of applying for your credential, CPR needs to be valid.

 

When you have finished completing the above, just scan all the documents into one PDF file and email to lucy.seegan@worldwidemarinetraining.com She will contact you after she has had a chance to review your materials and she will let you know if you need to send any further information.

Fees: $145 Coast Guard, $125 if we are to help you with your application. When Lucy contacts you that your application is ready to be sent, these fees can be paid over the phone with a credit card. The Coast Guard receipt will be attached to your application.
